Background:
Mr Smith’s rectal cancer was detected through routine screening. Mr Smith has a past medical history of proximal atrial fibrillation and hypertension.

Assessment Task
You are required to undertake a 30-minute Viva Voce to demonstrate your knowledge of caring for elective patients with complex care needs.

Marking Criteria:

This assessment addresses the following learning outcomes (LOs):

  1. Justify the selection and application of advanced monitoring, assessment and intervention techniques when caring for patients with increasingly complex needs, including non-scheduled and emergency, using a contemporary evidence base.
  2. Recognise the deteriorating patient and identify appropriate interventions for clinical emergencies.

As part of the assessment launch for this module, you will be given an SBAR handover of an elective patient with complex care needs. You will then use this SBAR handover to facilitate the first 10 minutes of your Viva, where using a contemporary evidence base you will discuss how you would prepare and care for the patient in the anaesthetic & post anaesthetic care phase. You should include:

  1. Consider airway selection, preparation & management
  2. Vascular access & homeostasis
  3. Non-invasive monitoring requirements
  4. Invasive monitoring requirements

After 10 minutes you will be asked to select a number between “1-7” which will facilitate the random selection of an update of the patient status including an ABCDE patient assessment. The patient presentation you are given will relate to one of the following clinical emergencies:

  1. Malignant Hyperthermia
  2. Can’t intubate/can’t ventilate
  3. Hypovolaemia
  4. Anaphylaxis
  5. Local Anaesthetic Toxicity
  6. Post-operative Myocardial Infarction
  7. Cardiac Arrest

You will be required for the next 20 minutes to process the ABCDE assessment (max 5 mins) before identifying and discussing how you would manage the clinical emergency from an anaesthetic and post-anaesthetic care perspective. You should discuss the following elements:

  1. A brief definition of the clinical emergency.
  2. An overview and rationale of the physiological changes which may be present due to the clinical emergency.
  3. Details of the interventions required and the rationale for these interventions based on the assessment.
  4. Consider what invasive monitoring may be used during the scenario and provide evidence-based rationale for this choice.
  5. Discuss the post-operative requirements for this patient.
