Ensuring all aspects of an individual’s health needs are met is critical to providing holistic care. In this assignment, you will broaden your knowledge of developing a plan of care. You will learn how to include an individual’s needs surrounding health literacy and social determinants of health (SDOH). Communicating these needs with interprofessional team members is a key component in advocating for the individual. This project provides a space for you to address this.

You will complete an individual’s scenario and develop a plan of care through a reverse case study. A reverse case study allows you to work backward from an individual’s outcome to explore and develop contributing factors and interventions within a plan of care. In this reverse case study, you will be provided with limited information about an individual. You will use critical thinking and your knowledge of holistic care delivery to fill in the gaps to complete the scenario. Use what you learned in the course and the module resources to complete the reverse case study. Review the provided information below and begin by asking yourself the following questions:

She states that although she has gained weight this past year, she has been feeling generally well, and she wants to make sure she is on track with her health, as she is expecting the arrival of her first grandchild. She expresses worry about her energy levels and occasional episodes of fatigue, which affect her ability to perform her job responsibilities. She states she earns minimum wage as a guide for a Native American heritage museum.
