According to the CDC (n.d.), “A community health assessment, also known as community health needs assessment, refers to a state, tribal, local, or territorial health assessment that identifies key health needs and issues through systematic, comprehensive data collection and analysis.” All local health departments are required to conduct a community health assessment.

Step 1: Choose a community health assessment. Select a county, hospital or community health center. Find their most recently published community health assessment. Identify one data source for each of the following topics/health indicator:

Infectious Disease

Chronic Disease

Behavioral Health

• Mental Health
• For example, for the “Infectious Disease” category, choose one infectious disease identified in the report. State the chosen infectious disease and provide an overview of the data or information you found in the report about the infectious disease. Search and find one data source where information about the selected disease you chose may have come from.
• The source of data may be on the local health department or county health department or county website. Keep in mind, some counties may use a dashboard, including this terminology may aid in the search. Refer to the topic materials for an example.
• Step 2: Create a 10-12-slide PowerPoint presentation that informs on each of the four indicators.

Use the following guidelines to create your PowerPoint presentation:

Introduction, including the link to the community health assessment.

Description of the health indicator from the community health assessment.

Summary of findings of the indicator as described in the community health assessment.

Discussion of the data quality and potential limitations of the data source.
