And, review the example paper. Your formatting MUST follow the format of the example.

You will review the three categories you assessed in your Community Health Assessment
Select ONLY 1 identified health-related weakness from the Community Health Category from the Community Health Assessment Guide (CAG) and describe the weakness (based on data). Do not use county data unless you have been permitted by the instructor to do so.
Allowable topics for your health-related weakness as supported by data the you collected for the CAG

This must be a chronic disease or condition identified as a leading cause of morbidity or mortality in your CAG

From the assessed categories Community People-Demographics, Physical Environment or Health and Social Services/Education, or from Further Information of Interest, identify and list:
Conditions in the Community (SDOHs) that have a connection to the weakness/concern
Third, draft the following sections for your paper based on the above analysis:
Introduction – Briefly describe your analysis process.
Insert the completed table in your written paper.
Analysis of Findings – Summarize the analysis of the identified health-related weakness with supporting details on how the data and conditions are related to the weakness with in-text citations.
Problem Identification- Use the following sub-headings and complete the following:
Assessment Data of Concern – Briefly describe the data and provide an in-text citation.
Example: XX% of African American males in XXXX (community) have been diagnosed with cardiovascular disease
Problem – State the identified health-related weakness
Example: There is a high prevalence of CVD in African American males > 65 years old in XXXX (community)
Aggregate – Identify the aggregate
Example: African American males > 65 years old
Evidence – State the evidence that indicates the concern IS a problem. This will ALWAYS be statistical outcome data. The outcomes that the problem “causes”.
Example: CVD in AA males results in XX ED visits and hospitalizations per year in XXXX (Community).
Conditions – Briefly describe the conditions and social determinants of health in the community that are related to the identified health-related weakness
Example: There is a high prevalence of tobacco use and limited access to health care in XXXX(community), and XX% of XXXX (community) experiences economic instability with limited options for healthy food consumption. In addition, there is a noted health literacy gap, most likely the result of educational attainment, language barriers, and lack of health promotion and health education in XXXX (community).

Please identify only 1 health-related weakness from your community assessment guide (CAG).
Do not use county-level data unless you have been given permission to do so.
The identified weakness needs to be health-related, and evidence must be available to support the problem assertion.
If you discovered a statistical health concern through your data collection that is not included in your CAG, you may use that, but it must be added to the Further Information of Interest at the bottom of the CAG and meet the criteria above.
Allowable examples of health-related concerns supported by statistical evidence are things like: heart disease, teen pregnancy, hypertension, stroke, specific type of cancer that has a higher prevalence than others in the community, mortality or morbidity from an infectious disease like influenza, HIV, COVID, Hep B, low rate of immunization, suicide, untreated mental health crises/concerns about mental/emotional wellness, depression and other mental behavioral health concerns.
The conditions and data must have a connection to the health-related weakness. In other words, what are the social determinants of health (SDOH) that contribute to the concern? These will always be SDOHs and/or conditions that exist within one or more of the SDOH categories. These are elements that fit into the categories of the Social Determinants of Health. For the table, you only need to list the conditions, but you will be required to make the connection to the SDOHs in your problem statement.
Keep track of any additional resources as you go. If you look up or add new information to your analysis and assessment, you will need to cite and reference those resources.
This assignment will set the stage for each of the remaining parts of this promotion: Planning, Implementation, and Evaluation. A clear and logically stated analysis and problem identification is the next step in successfully moving forward with subsequent elements of the community nursing process. Taking time to think through all the information you have will help with this part of the nursing process.
