Community Assessment

Overview
In this assignment, you take the next step in the nursing process, analysis and diagnosis/problem identification. You will use the assessment you did in Weeks 2 & 3 and build on it this week. Through the analysis of community assessment data, health promotion nurses take the first step in identifying who the various groups are within a community, including any possible at-risk aggregates. This analysis helps identify the weaknesses and stressors that are harmful to particular groups and to the community as a whole. Likewise, analysis of assessment data helps pinpoint social determinants of health that might be impacting the community in both positive and negative ways. Stating the identified trends, weaknesses, stressors, concerns, or conditions in the form of a community health problem helps to clarify the problem. It clarifies who receives care, states the specific problem this population is encountering, and lists the factors that are contributing to the problem. Having these factors clearly identified guides health promotion nurses in developing plans for problem-solving and health promotion.

Learning Objectives
In this assignment, you will have the opportunity to demonstrate the following learning objectives:
• Analyze community assessment data to identify strengths, weaknesses, concerns and/or conditions. (CO2)
• Formulate a community nursing diagnosis from data analysis. (CO2)

Instructions
First, review the instructional materials and the community assessment guide from Week 02 and the windshield survey from Week 03.
And, review the example paper. Your formatting MUST follow the format of the example.
Second, please use the following table and template to organize your analysis and diagnosis of the weaknesses, problem areas, strengths, and conditions in the community:Analysis & Problem Identification Template Download Analysis & Problem Identification Template
You will review the three categories you assessed in your Community Health Assessment
• Select ONLY 1 identified health-related weakness from the Community Health Category from the Community Health Assessment Guide (CAG) and describe the weakness (based on data). Do not use county data unless you have been permitted by the instructor to do so.
o Allowable topics for your health-related weakness as supported by data the you collected for the CAG
Infant Mortality Rate
Teen Birth Rate
Leading causes of poor health outcomes
 This must be a chronic disease or condition identified as a leading cause of morbidity or mortality in your CAG
 CVD
 Hyperlipidemia/Hypercholesteremia
 Hypertension

• From the assessed categories Community People-Demographics, Physical Environment or Health and Social Services/Education, or from Further Information of Interest, identify and list:
• Conditions in the Community (SDOHs) that have a connection to the weakness/concern
Third, draft the following sections for your paper based on the above analysis:
• Introduction – Briefly describe your analysis process.
• Insert the completed table in your written paper.
• Analysis of Findings – Summarize the analysis of the identified health-related weakness with supporting details on how the data and conditions are related to the weakness with in-text citations.
• Problem Identification- Use the following sub-headings and complete the following:
o Assessment Data of Concern – Briefly describe the data and provide an in-text citation.
 Example: XX% of African American males in XXXX (community) have been diagnosed with cardiovascular disease
o Problem – State the identified health-related weakness
 Example: There is a high prevalence of CVD in African American males > 65 years old in XXXX (community)
o Aggregate – Identify the aggregate
 Example: African American males > 65 years old
o Evidence – State the evidence that indicates the concern IS a problem. This will ALWAYS be statistical outcome data. The outcomes that the problem “causes”.
 Example: CVD in AA males results in XX ED visits and hospitalizations per year in XXXX (Community).
o Conditions – Briefly describe the conditions and social determinants of health in the community that are related to the identified health-related weakness
 Example: There is a high prevalence of tobacco use and limited access to health care in XXXX(community), and XX% of XXXX (community) experiences economic instability with limited options for healthy food consumption. In addition, there is a noted health literacy gap, most likely the result of educational attainment, language barriers, and lack of health promotion and health education in XXXX (community).
• Summary of Analysis and Problem Identification – Briefly describe the importance of this analysis and diagnosis with supporting details and in-text citation.
