For this assignment, you will begin exploring your population health intervention. You will identify a population health topic, related to a population of interest, and a devise an intervention. You will consider several content areas to frame your final assignment.
INSTRUCTIONS
a. Identify a population health topic, as it relates to a select population and is of relevance to your current healthcare experience.
b. Develop an APA style alphanumeric outline that includes the following content areas:
• Overview of selected population
• Disease-specific data
• Suggested intervention
• Setting
• Budgetary needs
• Funding sources
• Timeline for implementation
• Evaluation methods
c. Reference the Quality and Safety Education for Nurses (QSEN) competencies and identify those competencies (quality improvement, safety, informatics, teamwork and collaboration, and evidence-based practice) that would support/be relevant to the development and implementation of the population health intervention. Thread these competencies throughout the content of your outline.
d. Support your outline with evidence from 3 – 5 scholarly sources in addition to the course textbook. References must have been published within the past 5 years. The outline must be 2 – 3 pages (excluding the title page and reference page).
