The purpose of this assignment is to collaborate as a team to create an action plan for implementing a public health intervention within a selected community/culture.

Throughout Weeks 1-4 of the course, you will participate in a series of Collaborative Learning Community (CLC) assignments. These assignments will build upon one another and be used as the basis for the Reflective Journal assignments throughout the course.

As a public health professional, it is important to align public health programs with an organizational mission, vision, and objective to ensure sustainability and growth within the target community. Using the feedback you received from your instructor about the intervention you submitted in the Week 2 assignment below in bold, collaborate as a team to discuss your vision and the leadership and management approach you think will be most effective for implementing the intervention. In a 500-750 word document, succinctly state the following:

• Mission
• Vision
• Objectives
• Detailed action steps for implementing the intervention
• Description of how continuous quality improvement (CQI) tools or techniques can be used to enhance this plan
