My project is focused on whether motivational interviewing (MI) as an adjunct to diabetes treatment can have a positive effect on HbA1c levels, as opposed to standard diabetes treatment. My population group for this project is African-American women. This is important because diabetes have reached epidemic proportions in this population group and despite standard diabetes treatment, HbA1c levels in many, remain uncontrolled. Unfortunately, this can lead to Heart disease and a higher risk for heart attack and stroke. This can also lead to kidney disease and blindness. Motivational interviewing is a collaborative counseling strategy designed to elicit and strengthen motivation for behavior change. When MI is implemented studies have shown better health outcomes in populations with chronic diseases (Rollnick & Miller, 2008).
The proposal will be presented at one of the largest physician-owned primary care medical groups in central Ohio. This facility operates on a Patient Centered Medical Home (PCMH) model of care. If the proposal is accepted then the program could be implemented as a pilot in one office and hopefully based on results it will be implemented in other offices.
My PICO question for this project is as follows:
How does motivational interviewing as an adjunct therapy to standard diabetes treatment compared to non- adjunct therapy affect HbA1c levels in African-American women?
Below you will find the INSTRUCTIONS for the Evidenced Based Practice Proposal:
- The book you want to use to help you with this paper is as follows:
Translation of Evidence into Nursing and Health Care by Kathleen M. White
- Keep in mind that this is only a proposal!
- My PICO question is as follows:
How does motivational interviewing as an adjunct therapy to standard diabetes treatment compared to non- adjunct therapy affect HbA1c levels in African-American women?
- In these paper, use the IOWA model.
Evidenced Based Practice Proposal Instructions:
Develop a proposal to make a change in clinical practice that reflects the best research evidence available. The proposal must include:
- Introduction
-
- Background of the problem and significance to nursing.
- The project’s fit with the nursing strategic plan in the institution for which the project is proposed.
- A description of what led to your interest in this clinical question.
- A clear PICO(T) question
- Critical appraisal of relevant literature
- . A flow chart of the search strategy used and results
- A description about how you made decisions regarding which articles to include in your final project.
- A synthesis table that describes the level and strength of evidence used.
- Summary of literature with a focus on the implications for practice.
- A narrative description of your recommendations based on relevant literature. The literature included must be specific to your stated outcome measure.
- A description of the EBP framework used to develop the proposal (JBI, ARCC, IOWA Model, etc.) and how the model will guide the implementation of the project.
- Project Plan
- . Include time frames
- Key players to involve
- Administrative support needed
- Project roll out plan
- Identification of potential barriers and facilitators to project implementation
- A discussion of strategies to mitigate barriers and leverage facilitators.
- Assess and describe the feasibility of the project.
- Plan for sustainability of practice change.
- Evaluation Plan
- . Be specific, how will you know the practice change was successful?
- Include cost analysis if appropriate.
- Dissemination Plan
- . Describe your plan to disseminate your findings.
