J.S. is a 33-year-old Male seen with history of opioid dependence who comes to the office today stating, “I have been taking Roxy 30 mg for a very long time and have researched various things to substitute/ and be able to come off of the pills.”
Patient reports that addiction started after surgery related to gun shot in 2007.
He was prescribed Percocet and when he could find it, he would go after roxy on the street. Now he is taking Roxy 30 mg about 5 to 6 times a day. Patient reports that because of the addiction he lost his business and his relationship with his family is strained. He reports that he would get violently sick if he does not use Roxy.
He reports that it has been a year since he got out of prison due to drug related charges and probation violation.
He reports that he constantly in pain, but he would like some help to get off of the street drug. He denies any prior psychiatric hospitalization. Has been to rehab once after a court order.
Patient denies any recent period of abnormal or persistently elevated, expansive or irritable mood, any engagement in high-risk behaviors, sexual promiscuity, decreased need for sleep, racing thoughts, flight of ideas, increased talkativeness, pressured speech, indestructibility and lack of impulse control. Denies any psycho-motor agitation.
